# Insurance claim coverage summary

## Purpose

Make the factual and contractual basis of a claim review easy to inspect without presenting an unsupported coverage conclusion.

## Scope

Claim notices, policies, endorsements, correspondence, reports, invoices, and loss evidence. **Excluded:** final coverage decisions, legal opinions, settlement authority, and claims handling actions.

## Data basis

- Claim file, policy and endorsements, loss reports, expert documents, correspondence, claim history, and coverage-review guidance.
- The fixed records, tables, or folders in the skill scope that hold the relevant history or schema.

## Result

A claim chronology, coverage memo, evidence index, and open-points sheet with confidence and citations.

## Quality criteria

- Every material fact and coverage observation cites a file document and page or section.
- The chronology distinguishes reported, observed, and inferred events.
- Applicable insuring clauses, exclusions, conditions, and limits are separately identified.
- The memo states uncertainty and does not convert missing evidence into a denial.
- Open points identify the document or fact needed to assess them.

## Instructions

Read the policy schedule, wording, and endorsements together. Separate facts, policy text, interpretation, and recommendation. Test each potential coverage issue against the evidence and identify competing readings where wording is unclear. Quantify amounts only when the file supports them. Use the firm’s confidence vocabulary and make the qualified reviewer’s role explicit.

## Adapt before use

Keep the source trail close to every conclusion and preserve the difference between a source fact, a calculated value, an assumption, and a recommendation. Use stable identifiers across the document and sheet so a reviewer can move from a summary statement to the underlying row and source. Prefer a complete, transparent partial result over a confident answer built on missing evidence.
